    Ensocare Respond: Mobile Access to Patient Referral Requests

    Post-acute providers can respond to hospital referral requests from Ensocare Transition from virtually anywhere using a smartphone or tablet and Ensocare’s mobile application, Ensocare Respond.

    How it works:

    • When a hospital submits a referral request using Ensocare’s Transition discharge tool, a push notification is sent to the post-acute provider that a patient placement is available.
    • Swipe right from a mobile smartphone or tablet to directly view a high-level overview of the patient’s status.
    • See the referring hospital's name, location, patient’s clinical status, discharge notes and pertinent documentation associated with the referral—in a secure, HIPAA-compliant format.
    • View and respond to the referral request directly from the mobile app, accepting or declining the request.
    • If accepting, choose the date range you can accept the patient, as well as send notes back to the hospital discharge planner.
    • Review and confirm the referral response before sending.
    • The status is synced and reflected in the online provider portal for other members of the care team to view.
