(Omaha, NE) Ensocare, a leading provider of technology-enabled care coordination solutions, announced today the addition of Haugen Consulting Group, a leading health information management consulting firm, as a partner.

The move is expected to support and drive customers’ utilization of Ensocare’s unique Transition software solution that fully automates the patient discharge process. Transition allows healthcare providers to streamline operations, achieve cost efficiencies, increase patient satisfaction and reduce length of stay.

According to Ensocare President and Chief Executive Officer Luis Castillo, “We’re excited to bring our combined strengths to the provider marketplace,” he said. “Our technical expertise combined with the Haugen Consulting team’s deep understanding of the complex issues surrounding patient access and healthcare IT management, provides hospitals with the complete package needed for operational success.”

Mary Beth Haugen, Co-Founder and CEO of Haugen Consulting Group, added that the newly formed partnership capitalizes on the strengths of each of the respective companies. “Our partnership with Ensocare will give Haugen Consulting even deeper insight into the challenges involved in streamlining the care coordination process,” she said. “And I believe the insights we have about healthcare provider needs will inform the technology Ensocare develops.”

Ensocare’s solutions are geared toward increasing efficiency and coordination of care for patients both before and after discharge. According to Castillo, hospital patients and their families are often faced with having to select a nursing home or post-acute care facility quickly, during an already stressful time, with very little information.

Ensocare’s unique technology solution, which can be used by hospital discharge planners and case managers at the bedside in conjunction with all leading EHR and EMR systems, quickly matches the patient’s clinical needs with the capabilities of post-acute care facilities nationwide. Ensocare’s leading-edge technology is strengthened by its exclusive, national, no-cost network of post-acute care facilities.
